This documentary series investigates aviation accidents and how the elements contributed to them. Season 2 begins with a look at cockpit communication and how it improved with the implementation of Crew Resource Management (CRM) in 1979. The effects of storms, high winds and microbursts on planes are examined.

Instances of onboard fires are examined. Over the 43 minute program In some episodes a survivor or survivors would explain what they experienced during the accident.
